| The FALCON Club donates money to local sports entities and other groups in need. To request a donation, please fill out the form and you will be contacted. In order to be considered for a donation you will be asked to attend a monthly meeting and present your request, in person, to the members in attendance. |

| Coaches, if you would like to nominate one of your football players for the "Player Of The Week" award, submit this form. |

| If you would like to nominate someone for Youth Sports Coach of the Year, please complete this form (Word format) and mail to the FALCON Club or email the form to any Falcon Club Officer prior to December 1. |
